Back in November 2002 Michelle Branch's "Goodbye to You" spent 3 weeks at #2 behind her and Santana's "The Game of Love" then after "Goodbye to You" fell from the #2 spot Santana/Chad Kroeger's "Why Don't You and I" occupied that spot for several weeks before taking the top spot for 2 weeks. Those two flip-flopped once more after that. I believe that is the only time that has happened and is strange how they were connected.
My current chart has Default at #2 and #3.

On MT40, in addition to Chantal doing it in 1999 and Avril in 2004, the Dixie Chicks did it (I think) in 2006. I'd have to double check but I think 'Not Ready To Make Nice' and 'Voice Inside My Head' held the Top 2. I don't think it's happened since though. It's more and more difficult lately to have two simultaneous hits rising or maintaining high positions on my chart due to lots of competition.

5 Seconds Of Summer occupied not only the top two, but the entire top FOUR positions simultaneously during the weeks of September 13, 2014 and September 20, 2014 with "She Looks So Perfect", "Don't Stop", "Good Girls" and "Amnesia".
5SOS and Jamie Foxx are the only artists ever to occupy the top 3 (or more) simultaneously; Jamie Foxx did so first on May 30, 2009, before 5SOS did it again on September 6, 2014 (the week before "Amnesia" reached #4).
There have been at least a dozen occasions where the same artist has occupied the top 2 positions, so I don't consider it much of an anomaly.
